Parent Meeting: Nov. 26 at 6:30 at the Tempe YMCA - Teen Center
Please bring membership form, medical release, 2 copies of birth certificate (if not already on file), Parents Code of Conduct (signed by both parents), first payment of $225 (checks made payable to Jennifer Keller), sizes for shorts and jerseys.

2007/2008 Tryouts and Required Forms
Tryouts: Nov. 18th from 4:30 -6:00pm at Tempe Multigenerational Center.
Membership forms need to be filled out. Please print and fill out and bring to tryouts Nov. 18th but please do not sign and date until Nov. 18th.
2008 AZ Region Membership Form - 2 pages
2008 Medical Release Form - new 1 page
Parents Code of Conduct - *Parents will be required to read and sign.
